Another Ambulance at Destinations to Recovery Canon House

On October 17, 2016 there was another ambulance called to Destinations to Recovery Canon House in Topanga Canyon, California to respond to another situation at the Canon Drive house. This latest incidence happened at the Canon Drive facility at approximately 9:20 in the evening and eventually, the private ambulance company left at approximately 10:00pm. Regarding this most current incidence at least one client was taken away via ambulance. It was probably a suicide attempt. The residential is not a hospital, and if clients are suicidal, they need to be evaluated. Doesn't mean place is unsafe

Sexual Abuse at Destinations to Recovery Canon House: Substantiated

The following is a portion of the State of California Community Care Licensing (CCL) report and their concerns regarding Destinations to Recovery Canon House. "As a result of substantiating that a staff person had sex with a minor client in Complaint Investigation Control Number 34-CR-201602****3114, immediate civil penalties in the amount of $2,500 will be assessed for a violation that resulted in abuse of one or more clients." During this meeting Destinations to Recovery Canon House attempted to change the wording on the report from "sexual abuse" to "sexual contact with a minor" because Destinations to Recovery Canon House said that the minors consented to sexual acts with staff in the facility. Quoting directly from the report it states that, "Licensees* expressed concern that the wording within a citation dated 06/20/2016 appears to lead one reading the report that "sexual abuse" occurred, when minors consented to sexual acts with staff in the facility; licensees would like the wording to be altered to express that "sexual contact with a minor" was committed. The licensees stated that they believe the Type A citation should be a Type B citation, as the immediate threat was removed as soon as reasonably possible. RM explained that the language in the regulations and the type of violation that occurred per regulations fall under strict guidelines and cannot be altered by a regional office. Per the new Enhanced Civil Penalty laws, Health and Safety (H&S) Code section 1548(d), the Department shall issue a civil penalties for a violation that the department determines constitutes physical abuse, as defined in Health and Safety Code section 1548(e)(2), including sexual abuse as defined in Section 11165.1 of the Penal Code. As a result of substantiating that a staff person had sex with a minor client in Complaint Investigation Control Number 34-CR-201602****3114, immediate civil penalties in the amount of $2,500 will be assessed for a violation that resulted in abuse of one or more clients." This website is absurd. The creator of this website is a disgruntled neighbor to one of the houses run by Destinations. As a member of this neighborhood, we are completely disgusted by this woman. She is a disgrace to our community. This house causes absolutely no disturbances, and we as a community are thrilled to have them in our neighborhood. The staff are wonderful and many of the comments on this site are straight up lies. The woman running this and writing the reviews is an embarrassment. She has no compassion, no ground to stand on and absolutely no class. She spends her days screaming at those coming from and going to the house.

I agree with this assessment, and my daughter was a resident there. The next-door neighbor yelled at us while we were getting into our car, and my daughter said she once threatened the other residents with a gun when they accidently walked near her property.

I know it must not be easy to live near troubled teens, but know they are exactly that- "troubled".

They have a mental illness and need help and compassion. All the resident (teens) in the house while my daughter was there were respectful.

What did the police say when your daughter them that the neighbor once threatened the other residents with a gun when they accidently walked near her property? That is very scary.

Why would Destinations to Recovery stay in this house with such a crazy neighbor? She sounds addled and very aggressive.

To state the community is thrilled with a 6-bed group home with substantiated concerns by DCFS and Sheriff's Department about client safety is simply not true. This is not consistent with the values of our community in Topanga.

To further degrade a woman as part of their defense speaks volumes.

Something is not right with her.

I was not beeping anything. She sits on her porch and glares at anyone that comes close to her house.

Destinations to Recovery Needs Better Supervision

Much of the supervision at Destinations to Recovery is evidently provided by trainees or volunteers, and they are wholly inadequate to keep these kids in line once they become unruly. There were numerous police visits during my son's stay. Additionally, there was a lot of sex going on that staff either didn't know about or didn't want to know about. The entire program was disorganized and everyone seemed to be flying by the seat of their pants. There was also a lot of turn over. My son had two different therapists leave during his 45 day stay. The third therapist met with us for a family session, confused us for another family and kept pushing issues that weren't ours. When she realized what she had done, she apologized, but the session was wasted and only added to the confusion that hung over the entire experience. Had we not enrolled him elsewhere to get his issues addressed (and being new to using treatment facilities), I might never have known how bad the treatment was.

Los Angeles Sheriff's Department Police Cars at Destinations to Recovery Canon House

On August 30, 2016 the Los Angeles County Sheriff's Department was called to Destinations to Recovery Canon House to respond to a situation from inside the home. Upon their arrival to the Canon House in Topanga Canyon, California authorities including police cars and fire trucks had to assist with an issue from inside the home in which at least one person from Canon House was involved. Los Angeles County authorities were at the facility for at least forty five minutes with full emergency lights on and the authorities had to enter the home to address and to help and provide a solution to the situation.

State of California Department of Social Services: Destinations to Recovery - Canon House ~ $1,200.00 Civil Penatly Assessed

Destinations to Recovery ~ Canon House was issued a civil penalty of $1,200 for failure to disclose that Canon House had changed their administrator of their facility. The reports states in short, "During this Informal Meeting LPM, Turner-Marshall discussed Mr. Livengood's qualification to be administrator for the above facility. Mr. Livengood and Mr. Stark* were informed of the importance of reporting to CCL** when there is a change in administrator/or social worker staff by completing and updated LIC 500 and submitting required documentation for CCL review and approval." In addition, "Mr. Livengood was informed that and administrator certificate is also needed and that all documents sent to CCL pertaining to administrator changes need to be present in staff file and available up on request by LPA. Licensees informed to schedule and attend Comp III April 14, 2016. In addition, a $1200.00 civil penalty has been assessed for failure to correct violation 84061(f) which states-the licensee shall notify the department, in writing with ten (10) days, of the hiring of a new administrator." Also, "Licensee failed to notify CCL of change in Administrator, even after being notified by phone, deficiency citation and email. The licensee failed to comply with plan of correction for deficiency cited on February 19, 2016; therefore the civil penalty was assessed." Destinations to Recovery: Canon House Serves Ages 13-17 / Not 12-18 Years Old As Per Their Website

Please note that Destinations to Recovery Canon House has a license for a Group Home serving children and adolescents between the ages of 13 and 17 , ambulatory only . The Destinations to Recovery website states that, "Canon serves adolescents 12-18 years old." I called and asked them about this. Apparently there is a difference between their residential and day programs regarding the ages that can be treated.

The state of California's Department of Social Services intervened in May of 2016 and had Destinations to Recovery change their website regarding the age of the clients they treat. As of May 29, 2016 the age reference on the Canon House website is in compliance with their California Group Home license - which it was not before.

Arrest Made at Destinations to Recovery Canon House

On Thursday - May 12, 2016 the Los Angeles County Sheriff's Department was called to Destinations to Recovery Canon House. As a result of this phone call, 3 Los Angeles County Sherriff's department police cars with approximately 4-6 officers arrived to investigate this specific incident. The sheriff's department stayed at the facility on Canon Drive for approximately 3 hours. And as a result of these actions a female client was taken away in handcuffs by the police. If you have further questions contact the Los Angeles County Sheriff's Department Lost Hills Station to find out more information. However, if the client is a minor the sheriff's department will not give you specific details.

And, by the way, there was an arrest when our child was there too. A young man whose parents put him in recovery to keep him from being arrested for a rape charge. It, obviously, didn't work.

How is your child doing now? Our child is back in another rehab facility and it's been less than three months.

She is 10 times worse than when we brought her in for 90 days awhile back. They have destroyed the fragile dynamics of our family that we did have and replaced it. With nothing.

I truly hope your family fared better with your troubled teen. I'm disgusted with them all there.

I am sorry you had that experience. My daughter was at Canon (which is not a rehab facility- Cheney is the rehab facility so you might be confusing it. She has been with Destinations after residential and is doing great

This sounds like a complaint from someone trying to slander this companies name and not a true reflection of services rendered. Maybe a competitor or the not all there neighbor who yells when I have to lock my car, resulting in a horn beep while on a visit to see my son?

I had a pleasant experience with everyone at the Canon location and would recommend it to anyone looking for a good program for their son/daughter.

As of March 23, 2016, Destinations to Recovery - Canon House has earned 5 Type - A Citations since it opened in 2014. According to the State of California's Department of Social Services website a Type - A citation, "It is for the most serious type of violations in which there is an immediate risk to the health, safety or personal rights of those in care. Examples may include lack of care or supervision, access to open bodies of water, lack of a fire clearance for the building and access to dangerous chemicals. Citations for these violations will always be issued even if the violation is corrected on the spot." In addition, Destinations to Recovery - Canon house has also earned a Type - B citation which the state of California's Department of Social Services states in their website as, "A Type B citation is for a violation that, if not corrected, may become an immediate risk to the health, safety or personal rights of clients. Examples include faulty medical record keeping and lack of adequate staff training." I just looked it up. Only one of the citations was found to be true.

I called and the state and found out that they left cleaning supplies unlocked. My kid has no interest in cleaning so this is not an issue for our family.

It is not all about you and your child's needs but the needs of all the children being treated. A clear example of the apple doesn't fall far from the tree here.

I just gotta say, this place saved me. I hated it as first cause who doesn't want to be at home doing whatever they want. But really I learned so much and couldn't be more tankful. I mean I learned about me, and my family and what I can do to become the person I actually want to be and not just who I thought I was stuck with. Its crazy to think of where I am now versus where I was going. And now I can talk to my family and not just keep secrets from them. My advice is to check this place out, it saved me and everyone I knew when there. Shout out to DTR!!

Do not send ANYONE (you love or don't) to this facility. My son stayed at the Canon house and it was complete chaos. The staff is very inexperienced and may have an average age of around 25. It is almost as if they hire staff right out of college with little to no experience. I felt that the staff was there just to collect a pay check and cared very little about actually helping my son. There was also no structure to the day and my son said that kids talked back to the staff and were disrespectful. Destinations to Recovery tells you what you want to hear via their website and the face to face contact just to collect from you and your insurance company. It is a complete rip of and I am out a ton of money which would be okay if they helped my son. Also, the house smells like SEWER! Clearly the house's septic system is failing. During my son's stay, a septic pump truck pumped the septic 3 times! Unbelieveable!

Wow... and you hired this person to work with teenagers with a multitude of problems? You hired a person with anger issues? Maybe you need to take a look at yourselves for hiring a person who feels, "Wronged" and could come back and possibly harm others. It is shameful that your company is running a facility that is supposed to help the future of our country yet you put them in harms way. Also, as per this comment, "Wish the best for her and hope she can overcome this deep seeded anger she has towards herself," sounds very insincere because if you really cared for this employee, you would never have written that comment in such a vindictive way. And if this is how you treat your clients, you should be shut down. Shame on you Destinations to Recovery~Canon House.

How Destinations Helped Me

Like any client sent to a residential facility I hated it and wanted nothing more but to leave. For parents, the progress of your child's treatment and recovery is solely on them. The staff members at Destinations are there to encourage your child and be that ear when they feel the need to be heard. A therapist and psychiatrist and any other mental health doctor is there to help guide your child. They are not there to "fix" your child. Of course this is what many parents want, it's what they expect. My parents are the fixing type, you tell them what's wrong and how to fix it and they will but that isn't how it works. Through family therapy at Destinations we learned not only about each other but ourselves and our roles in the family. I was in residential for a little more than two months then continued with the IOP program for about eleven months. Yes, it took a very long time for me to recover but everyone goes at their own pace. I have no one else to thank but Destinations and the team there for directing me to the place I am in right now. I loved the staff, I loved the feeling I got from them and the other clients. Most of the staff is young but I do know they have experience, maybe not as much as someone older but teens are able to relate to them easier, they feel much more comfortable with them instead of an elder. I love Destinations and so does my family. It helped me save myself.

Unregulated industries that are approved for submission by government committees into neighborhoods. They do this silently not asking for any neighborhood debate. The committee inserts a disruptive force into your home life without even due process. IF your teen were to be injured, or say a teen (under detox! an unknown outcome) were to get out and disrupt the area. Your home. Your neighborhood that you bought into and go home to relax. Has now been ripped from you for the profit of an individual that lives in a $26 million dollar Beverly Hills home. Behind 67 plus LLC's that basically limits that liability. On top of that the commission has inserted a high risk business into a neighborhood. So now its your problem and you had no idea. What if your kid is dies? What if your kid gets out and kills someone? Do you think that little LLC of one house is going to pay for the damages? YOUR COMMITTEE'S are not doing due process. Your voice isn't heard. What will it take to get these unregulated money making, insurance scam, for profit off your kid backs destructive commissions to come to their senses? A national news event? Keep in mind these are residential houses.... NO ADA...NO outside regulatory entrance is allowed since these facilities hide behind the privacy rules.... These are businesses. They run 3 shifts of at least 100 car impacts a day. They ruin neighborhood life for the benefit of one person... a for profit business. They take your taxes. They take your money. They suck the life out of everyone in a residential setting. Around the clock noise... 3 shifts... multiple van deliveries (they bring kids from other locations). All of this.... brought to you by your friendly neighborhood commissions. The state is picking up all liability since each LLC is limited to its exposure.... protecting their assets. However the commission took your liability away and exposed you to a greater amount of risk. Disgusting that you come to America to build the dream, work hard to build a home, and a commission that doesn't even inform the public, decides your fate without a word to you. Really LA County? Really State of California? DESTINATION TO DISCOVERY - BAD BUSINESS, BAD LIABILITY, EXPLOITING YOUR PAIN FOR PROFIT, YOU HAVE BEEN WARNED DOING BUSINESS WITH THEM.
